Safety Protocols Checklist for Ibogaine Clinics: Please check all that you have on your premises.

1. Pre-Treatment & Screening(Required)

a. Thorough Medical Prescreening : Comprehensive medical, psychiatric, trauma, and substance-use history.

b. Cardiac Screening: Baseline vital signs and 12-lead ECG required.

c. Professional EKG Review: 12-lead EKG must be reviewed by a cardiologist or a competent clinician (not an EMT) to identify cardiac risks.

d. Blood Panels: Routine testing, including electrolytes, conducted during prescreening.

e. Medication & Supplement Reconciliation: Mandatory review of all current prescription medications and supplements to identify drug interactions (specifically checking for CYP2D6 inhibitors and QT-prolonging drugs).

f. Cardiac Exclusion Criteria: Mandatory exclusion of patients with QTc intervals >450ms, history of serious arrhythmia, structural heart disease, or other significant cardiac risks (e.g., family history of LQTS/sudden death).

2. Clinical Oversight & Staffing(Required)

a. Medical Doctor (MD) On-Site : A doctor must be on premises, overseeing the 24-hour flood-dose window and present during all Ibogaine sessions.

b. ACLS Certification : The overseeing Medical Doctor must be certified in Advanced Cardiac Life Support (ACLS) protocols.

c. Iboga/Ibogaine Experienced Practitioners : Experienced staff must oversee the session.

d. Dedicated Support : A dedicated nurse or support staff should monitor the patient (e.g., sitting for at least 18 hours per patient).

3. Emergency Preparedness & Monitoring(Required)

a. Continuous Real-Time Monitoring : Ongoing EKG and blood pressure monitoring.

b. Defibrillator (AED) & Oxygen : On-site and immediately available during the entire flood-dose phase.

c. Emergency Equipment Kit : Must include suction unit, IV cannulation kit, airway adjuncts, and emergency medications (within scope of practice).

d. Emergency Support Team : Paramedics and an ambulance must be on-site (or a pre-defined, verified hospital transfer pathway must be established).

e. Proximity to ER : Hospital/Emergency Room must be within a realistic transfer time (typically targeting <15 minutes).

4. Coaching, Integration & Governance(Required)

a. Preparation and Integration Coaching : Options for pre-treatment preparation and structured integration provided.
